Based on member feedback, ASTRO is urging hospital systems and freestanding centers that have not already done so to reach out to their contracted commercial payers and update their chargemasters or renegotiate rates for the new treatment delivery codes, CPT 77402-77412. 

Commercial contracts referencing old CPTs could result in underpayment unless renegotiated or interpreted via crosswalks. Updated rates should reflect that the technical component of image guidance, CPT 77387, is now bundled with the delivery codes. It is likely that many payers are still reimbursing based on old, contracted rates for 77402-77412 but those codes have been overhauled, and older rates do not reflect the new valuation of the codes.
