ASTRO releases Active Motion Management Model Policy to support members

ASTRO has heard from members across practice settings who are experiencing uncertainty and administrative challenges when billing for Active Motion Management (AMM) under the 2026 radiation oncology CPT code changes. Members have reported denials from payers and questions from billing teams about when AMM is appropriately reported, how the revised treatment delivery code structure should be interpreted, and what documentation is needed to support medical necessity.

These issues have created operational burden for practices already adapting to significant changes in radiation treatment delivery and image guidance coding. Members have raised concerns that clinically appropriate use of AMM may be misunderstood during prior authorization submission and payer review, leading to treatment delays and denials.

To help address these concerns, ASTRO has developed an AMM Model Policy that describes coverage and documentation expectations for AMM during external beam radiation therapy. The policy is intended to support consistent communication among clinicians, billing staff, practice leaders, and payers when AMM is clinically appropriate and supported by the patient’s treatment plan and documentation.

ASTRO encourages members to review the policy and share it with their internal teams as they continue implementing the 2026 coding changes. ASTRO will also share this resource with insurance payer contacts to promote awareness of the clinical and documentation considerations that support appropriate AMM billing. While final coverage decisions remain subject to each health plan and patient-specific circumstances, ASTRO hopes this resource will help reduce confusion and support appropriate access to AMM when medically necessary.
