ASTRO raises RT reimbursement concerns with Aetna and Cigna
October 28, 2026
ASTRO recently sent letters to Aetna and Cigna urging immediate correction over ongoing reimbursement issues related to the radiation therapy treatment delivery coding changes that took effect on January 1, 2026. Although these changes have been in effect for nearly a year, providers continue to report that Aetna and Cigna have not fully updated their reimbursement methodologies, fee schedules, and claims processing systems to reflect the new coding structure. As a result, radiation oncology practices across the country are experiencing claim denials, payment delays and increased administrative burdens. ASTRO urged both insurers to take immediate action to update their systems and ensure accurate reimbursement for radiation therapy services. ASTRO noted that continued reliance on outdated payment structures creates significant financial and operational challenges for providers and may ultimately affect patient access to timely, high-quality cancer care. ASTRO emphasized the importance of consistent implementation of the 2026 coding changes and called for prompt resolution of the reimbursement issues facing practices nationwide.
