ASTRO is pushing back against EviCore’s outrageous updated guidelines for Radiation Oncology services that would limit coverage for complex treatments. Effective January 1, 2026, EviCore plans to restrict billing of CPT 77412 (Level 3 radiation therapy) for nearly all clinical indications. However, Medicare anticipates that approximately 35% of radiation treatments will be billable under the Level 3, highest complexity code, including breast cancer and lung cancer cases that use active motion management to ensure safe, effective treatments. ASTRO has already begun efforts to curb implementation of this policy as is.
