On November 21, 2025, CMS released the 2026 Hospital Outpatient Perspective Payment System (HOPPS) final rule, which takes effect on January 1, 2026, and includes a 2.6% increase factor and responds positively to ASTRO recommendations for using appropriate cost data for reassigning revised radiation treatment delivery codes to ambulatory payment classifications (APC). As finalized, 77402 will be in APC 5621, 77407 will be in APC 5622 with a significantly higher rate than proposed, and 77412 is assigned to APC 5623. CMS also finalized payment rates for new superficial radiation therapy codes.
