On November 1, the Centers for Medicare and Medicaid Services (CMS) released the 2025 Medicare Physician Fee Schedule (MPFS) final rule that includes payment reductions for key radiation therapy codes. The Conversion Factor (CF) for 2025 will be $32.35, which is almost a 3% reduction from 2024’s final CF of $33.29. CY 2025 also marks the final year of the four-year phase-in of the Clinical Labor Price update, which lowers payments to specialties that use expensive equipment, such as radiation oncology, in a budget neutral environment. CPT Code 77427, Radiation treatment management 5x, which has been on the Medicare Telehealth List since the onset of the COVID-19 pandemic, will remain on the Telehealth List on a provisional basis.

These continued cuts underscore the need for the Radiation Oncology Case Rate (ROCR) Act (S. 4330/HR 8404), which is supported by more than 70 oncology organizations and would secure stable payment rates, improve upon already excellent quality and reduce disparities.
