This guideline will replace the original ASTRO guideline on this topic that was published in 2011 in light of the considerable new evidence that has accumulated in the intervening years. The guideline will cover five key questions regarding whole breast irradiation fractionation and technique. It first offers recommendations on preferred dose-fractionation for patients receiving WBI with and without a tumor bed boost and when a boost is indicated. It also considers how tumor and patient characteristics impact these questions. The guideline then evaluates the preferred treatment planning techniques for WBI and for tumor bed boost.
