RO-ILS Overview

The mission of RO-ILS: Radiation Oncology Incident Learning System is to facilitate safer and higher quality care in radiation oncology by providing a mechanism for shared learning in a secure and non-punitive environment. As the only medical specialty society-sponsored incident learning system for radiation oncology, RO-ILS will provide an opportunity for shared learning across all radiation oncology institutions, which is an added value to institutions that may be tracking incidents independently.

PQI Project Overview

This project outlines how participants can use the RO-ILS: Radiation Oncology Incident Learning System towards fulfilling the Part IV Physician Quality Improvement (PQI) Maintenance of Certification (MOC) requirements that have been established by the American Board of Radiology (ABR). To begin, interested participants should download the free template.

This project is based on the ABR’s four-part Plan-Do-Study-Act (PDSA) process for continuous quality improvement. Participants will link two PDSA cycles together to create an action-oriented improvement plan that assesses the effects of their quality improvement strategy. For the purposes of this project, each of the two consecutive PDSA cycles will coincide with two consecutive PQRS reporting period.

  • In the first PDSA cycle, participants will enter event data (an incident that reached the patient, a near miss, or an unsafe condition) using the RO-ILS portal, after which members of the treatment team attend a meeting to discuss what led to the event, identify areas of improvement, and develop an improvement plan. The improvement plan is then implemented. 
  • In the second PDSA cycle, a follow-up meeting is held to evaluate the implementation of the improvement plan and determine if additional actions are needed. The project concludes with a written participant reflection.

How to Participate

  1. Register for and download the free RO-ILS PQI template.
  2. Save a digital copy of the template in a location that you will have continual access to throughout the entire length of your project (typically a minimum of one calendar year).
  3. Read through the entire template to familiarize yourself with the project outline as well as the Radiation Oncology Incident Learning System reporting process.
  4. Sign up to participate in the Radiation Oncology Incident Learning System.
  5. Complete the first PDSA cycle by entering event data into the RO-ILS portal, participating in a team meeting to discuss the event, identifying areas for improvement and develop a performance improvement plan.  
  6. Implement your performance improvement plan.
  7. Conduct a follow-up meeting to evaluation the implementation of the improvement plan and determine if additional actions are needed.
  8. Complete an individual participation reflection.
  9. Attest to your project completion on the ABR website.

Recording Progress and Reporting Completion

Participants are responsible for documenting their project using the outline included in the template. Participants will not submit their partially or fully completed template to ASTRO at any point during their project. Once completed, participants must personally attest to its completion on the myABR website in accordance with the ABR’s MOC requirements. While ASTRO is able to confirm a department's participation in the Ro-ILS, ASTRO will not be collecting, reviewing, or validating individual participant’s projects.


The Radiation Oncology Incident Learning System (RO-ILS) PQI Template is qualified for physicians and physicists by the American Board of Radiology in meeting the criteria for practice quality improvement (PQI), toward the purpose of fulfilling requirements in the ABR Maintenance of Certification Program. The activity was approved on April 14, 2014, and expires on April 13, 2020.
