ASTRO’s 2009 Annual Meeting will be held November 1-5 at McCormick Place West in Chicago. Those interested in applying for the Survivor Circle Award can fill out the application below and return it to the ASTRO offices by August 29, 2009. You must be a resident of the Chicago area and have been treated with radiation therapy to qualify.
