Press Kit: Radiation Oncology Payment Reform

Radiation oncology delivers high value for patients and for Medicare, but reimbursements for radiation therapy under the Medicare Physician Fee Schedule have been cut by 23% since 2013, more than nearly all other medical specialties. In 2023, ASTRO introduced the Radiation Oncology Case Rate (ROCR) program, a legislative initiative to provide relief from the ongoing cuts and ensure stability, access and equity in cancer care.

ROCR would:

  • Change radiation oncology payment from per fraction to per patient
  • Reverse decade-long declines in Medicare payments
  • Usher in a new era of stable payments, higher quality care and reduced disparities
  • Safeguard the ability for patients to receive cutting-edge care close to home

Radiation Oncology APM (RO Model)

In September 2020, the Centers for Medicare and Medicaid Services (CMS) issued a final rule establishing a Radiation Oncology Alternative Payment Model (RO Model); in August 2022, the agency issued a final rule that indefinitely delays the implementation of the model. ASTRO expressed serious concerns about the success of the RO Model given its incongruity with recommendations provided by radiation oncology stakeholders throughout the rulemaking process.
